The Wellness Group administers a discount medical plan (“Plan”), though which individuals who enroll in the Plan (“Members”) can receive professional healthcare services at discounted rates from a certain network of providers and other businesses who have agreed to participate in the Plan. Provider is a provider of healthcare services or a facilities that offers healthcare services, including but not limited to healthcare services, chiropractic care, physical therapy, counseling, primary care, imaging, and other items and services at discounted rates from a certain network of providers and other businesses who have agreed to participate in the Plan. Provider is qualified, by virtue of education, training, and experience to provide such items or services to Members. The Wellness Group and Provider want to establish a relationship such that Provider will participate in the Plan and offer discounts to Members for certain services in accordance with the terms of this Agreement.

As a Wellness Group Provider, your clinic is eligible to offer compliant cash rates, flat fees, or discounts. These discounts can apply to healthcare services, examinations, imaging, treatment, therapy, adjustments, manipulations, counseling, and/or supplements. The Provider chooses which services to discount and patients can then enjoy these discounts once they are signed up as a member of The Wellness Group through your clinic.

There is no cost to the clinic for this program.
